Pathophysiology

In hypothyroidism, decreased hormone production leads to compensatory TSH elevation, while hyperthyroidism involves excessive hormone production often with TSH suppression. Clinical Reasoning

Clinical presentation varies dramatically between hypo- and hyperthyroid states, with hypothyroidism presenting as fatigue, weight gain, cold intolerance, and bradycardia, while hyperthyroidism manifests as weight loss, heat intolerance, palpitations, and anxiety. Laboratory evaluation should include TSH as the initial screening test, followed by free T4 and potentially T3 levels based on clinical suspicion. Treatment approaches range from hormone replacement therapy in hypothyroidism to antithyroid medications, radioactive iodine, or surgery in hyperthyroidism, with monitoring guided by symptom resolution and biochemical normalization.
